Providence is calling a Social Worker (Days, 1.0 FTE) to work in PACE Redmond at Providence Elderplace in Redmond, Washington.
*This position is offering a $5,000 sign-on bonus to eligible candidates.*
As a member of the interdisciplinary team, provides social work services to participants in a manner which is consistent with the mission and core values of Providence Health System. Spends a majority of time in direct service activities within the Center, interaction with other team members, and ongoing assistance to participants to address psychosocial issues. Based on comprehensive assessments, provides perspective on the social, emotional, cognitive and cultural factors that impact the creation of an individualized integrated plan of care. Accesses community and financial resources for participants and assures continuity of care across the continuum. We focus on wellness and whole person care in collaboration with an interdisciplinary team of skilled PCPs and allied health care professionals.
In this position you will:
All employees of Providence ElderPlace-Seattle are required to comply with all PACE regulatory requirements established by the Federal and State governments. This includes: understanding his/her role in the emergency preparedness plan; adhering to the agency’s infection control plan; and promoting the Participant Bill of Rights and Responsibilities. Understands the basic procedures for receiving and documenting grievances in order to initiate the appropriate process for participant concerns. Operates in a manner consistent with the values and ministry of the Providence Health System.
Completes comprehensive social work assessment and periodic reassessment of participants, including mental status tool. Shares findings with other disciplines and referral agencies as appropriate.
Refers participants and their families/friends to appropriate community agencies. Serves as a liaison and advocate with such organizations to assure participants receive full range of services to which they are entitled.
Participates in the interdisciplinary team planning conferences to develop an integrated, individualized plan of care. Brings participant input into the care planning process.
Involves participant, family, and other caregivers in developing an individualized plan of activities, long-term goals, and short-term objectives.
Required qualifications for this position include:
Master’s degree in MSW from an accredited school of social work or equivalent educ/experience
1-year experience with the frail elderly
2 years of social work experience including effective work with geriatric age groups from diverse ethnic cultures
Washington State Agency Affiliated Counselor or LIACSW or LAASW or LICSW or LASW
Current CPR certification.
Valid driver’s license and automobile insurance.
