Admission Clinical Liaison

The Admission Clinical Liaison is responsible for creating and maintaining positive customer relationships to increase referrals for hospice and palliative care. This role includes working with facility staff, physicians, and other individuals to assess appropriateness for hospice, palliative care or other services within the service line, and facilitate coordination of these services. The Admission Clinical Liaison will ensure there is appropriate documentation for compliance with agency and statutory guidelines. Consultative functions include, but are not limited to: providing consultation and informational presentations regarding end of life care, palliative care and hospice care; sharing expertise through in-services on pain and symptom management; serving as a clinical resource and educator for physicians, facility staff, patients, families and community on hospice and palliative care or other issues in various settings and facilities. The Admission Clinical Liaison may also begin the admission process including obtaining consents, beginning patient assessment, discussing goals of care and collaborating with hospice care team.

Required qualifications:

  • Education sufficient for licensure

  • Current Washington State licensure in the applicable discipline (e.g., Nursing, Social Work).

  • Current State Driver's license.

  • Automobile insurance.

  • Current Healthcare Provider CPR certification.

  • 3 years experience in clinical discipline.

Preferred qualifications:

  • Bachelor's Degree in nursing or other clinical area related to service line offerings

  • Liaison, marketing or education experience.

  • Health care industry experience.

  • Epic EMR experience.
