Providence PACE Community Outreach Coordinator in Seattle, Washington
Providence is calling a PACE Community Outreach Coordinator (Hospital Administration, Day Shift, 1.0 FTE) to Providence ElderPlace in Seattle, Washington. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all – especially those who are poor and vulnerable .
The Community Outreach Coordinator is a new, exciting role within Providence PACE programs. The PACE team of health care and social service professionals and our affiliates provide comprehensive integrated care to our participants, virtually all of whom are both Medicaid and Medicare eligible. The role will be responsible for the development of relationships with diverse community organizations and community leaders within the PACE service area to build thriving relationships between PACE and the BIPOC community. Responsibilities include educating the public about PACE, organizing community outreach and promotional activities, and guiding interested elders into the intake process. The Community Outreach Coordinator identifies and cultivates new relationships and actions to support PACE to best meet the needs of diverse local communities. The Community Outreach Coordinator also identifies opportunities to support the organization’s goals for enrollment and evaluates results. The role will also work closely with the Chief of Health Equity to ensure an aligned community engagement strategy across all Providence Home and Community Care programs.
In this position you will:
Identifies and/or participates in relevant marketing, outreach, and community engagement activities with diverse populations.
Identifies opportunities to collaborate with community organizations, with specific focus on inviting organizations to lead relevant adult day activity events that will appeal to diverse populations, and to interface with operations and clinical disciplines to integrate culture competency into PACE services.
Identifies opportunities for community members and organizations to connect with PACE staff about their local needs, resources, and how PACE can best work to meet those needs and engage those resources.
Establishes and maintains relationships with community leaders.
Develops/participates in speaking engagement opportunities for this position and other members of the PACE team.
Provides education to referral sources and the community on PACE services.
Required qualifications for this position include:
Bachelor's degree in Marketing, Social Work, Public Health, Health Administration or equivalent educ/experience
2 years of experience with community-based organizations, social services and/or health care programs
2 years of experience in working with diverse populations
2 years of strong understanding of Diversity, Equity and Inclusion work
Preferred qualification for this position include:
- Prior experience in a similar workforce integration role
