Providence Health & Services Physical Therapist - Providence ElderPlace in Seattle, Washington


Providence is calling a Physical Therapist (PT) (Physical Medicine Clinic, Day Shift, 1 FTE) to Providence ElderPlace in Seattle, Washington. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all – especially those who are poor and vulnerable.

In this position you will:

  • Performs thorough PT assessments evaluating physical and functional mobility status of PEP participants including but not limited to ROM, strength, balance, sensation, coordination and gait. Determines need for PT interventions, caregiver training and durable medical equipment.

  • Develops plan of care in accordance with participant needs and team approval.

  • Interacts with the public and other employees in a positive manner.

  • Demonstrates an ability to treat a wide range of geriatric diagnoses and be knowledgeable in therapy treatments, goals and contraindications for treatments. Treatments are adapted to the unique PEP-S population. Requests consultation, supervision or continuing education when appropriate to learn specialized techniques or treatment approaches.

  • Provides participant/caregiver education and counseling regarding issues related to rehabilitation treatment and goals, safety risks and equipment needs.

  • Documents all PT evaluations and interventions in appropriate agency format and within agency timelines. Documents reflect good clinical documentation. Responsible for tracking and monitoring participant DME.


Required qualifications for this position include:

  • Bachelor's Degree in Physical Therapy

  • 2 years of related experience in rehabilitation setting.

  • Current licensure or certification in Washington State.

  • Valid driver’s license.

  • Current auto liability insurance.

  • Current CPR certification.

Preferred qualifications for this position include:

  • Masters or Doctoral degree in Physical Therapy from an accredited school

  • 2 years working with a geriatric population
