Providence is calling a Registered Nurse (RN) Case Manager to support PACE (Program for All-Inclusive Care for the Elderly, Day Shift, 1.0 FTE) at Providence ElderPlace in West Seattle, Washington.
PACE is the only program of its kind to deliver whole person-care for the elderly through a collaborative environment akin to family. This is an opportunity to join a unique, interdisciplinary team of medical, social, personal, and residential care providers practicing outcomes-based and integrated care that allows for positive life-changing decisions.
In this position you will:
Facilitate interdisciplinary team care coordination, intensive medical management of participants recently discharged from hospital and skilled nursing facilities.
Collect the necessary medical and social information to ensure comprehensive decisions are made which ensure the safety and well being of participants.
Act as a medical advocate for the participant, and coordinate care services with the Interdisciplinary team.
Build strong relations with participants, caregivers, families and community providers so that communication and coordination occurs in a manner that facilitates optimal care and financial oversight.
Required qualifications for this position include:
Graduate of accredited school of nursing
Current Washington State Registered Nurse (RN) license
Current Cardiopulmonary Resuscitation (CPR) certification
Current Washington State Drivers License
Proof of auto insurance
1 year of nursing experience
Preferred qualifications for this position include:
Bachelor Nursing Degree
Nursing experience in Medical Surgical or Rehabilitation
