Providence Social Worker- Long Term Care in Seattle, Washington


Providence is calling a Social Worker ( Long Term Care , Day Shift, 1.0 FTE) to Providence Mt St Vincent in Seattle, Washington. In this position you will:

  • Crisis intervention: Assess resident and family needs, counsel; work with staff to enhance understanding of emotions and relationships between residents, family members, and staff members; advocate, teach, and refer as needed.

  • Attend quarterly and annual care plans, facilitate meetings, and complete required documentation.

  • Provide consultation and advocate for residents with staff, family and administration.

  • Facilitate resident support and socialization groups and provide social work group to enhance relationships among residents and promote psycho-social functioning.

  • Provide one to one counseling to aid in adjustment to illness, coping with grief, loss and dependency issues. Provide emotional support to family members and to staff as needed.

  • Outreach and assessment: Welcome residents who have been directly admitted to the unit and assist staff in orienting the person to the unit as well as to the services of Mount St Vincent. Complete a psycho-social assessment by reviewing the medical record, interviewing resident and his or her “family” and support system, and contacting community agencies when warranted. Discuss and document choices about advance directives and CPR versus no CPR. Provide information about Power of Attorney when indicated. Aid in development of the initial care plan.

Required qualifications for this position include:

  • Bachelor's Degree Social Work Or

  • Master's Degree Social Work Or

  • Master's Degree In related field.

  • 2 years Experience appropriate to job responsibilities.

Preferred qualifications for this position include:

  • Geriatric and/or medical social work.
