Providence is calling a Licensed Practical Nurse (LPN) Care Coordinator (Hospice, Day Shift, 1.0 FTE) to Providence Hospice of Seattle in Tukwila, Washington.

In this position you will:
Implement nursing interventions based on plan of care and Hospice policies and procedures, including effectively addressing pain management issues; effectively addressing the management of other symptoms; providing active listening/support and education to patient and family regarding end-of-life issues.
Understand the fundamental differences between prescriptions and orders and the roles of each as they relate to the hospice organization, patient and scope of practice.
Maintain current Medication Profiles, Prescription Files and Patient Specific Prescription Files; take responsibility for understanding patients’ insurance benefits and managing care in a cost effective manner.
Provide medications in the most efficient and cost effective manner in accordance internal and external Medication Billing and Medication Procurement Processes.
Required qualifications for this position include:
Degree completed from an accredited Licensed Practical/Vocational Nurse program
Current Washington State professional licensure or certification
3 years nursing experience
Preferred qualifications for this position include:
Hospice certification or advanced certification
Prior Hospice, Homecare, Long Term Care, medical/surgical, and/or oncology experience
