Providence is calling a Pharmacist (1.0 FTE/40 hours a week, Day Shift) to the Patient Registration Department at Providence Infusion & Pharmacy Services in Tukwila, Washington.

This position is responsible for medication distribution and clinical drug services of IV therapies to Home Infusion patients, Specialty Infusions, Infusions at our Ambulatory Infusion Suites (AIS), Hospice Services, and Skilled Nursing Facility patients by providing the best possible drug therapy under the highest professional standards. The pharmacist works closely with nurses, dieticians and pharmacy technicians to ensure that all patients are receiving quality care that is delivered in a safe and efficient manner.
In this position you will:
Interpret orders and/or prescriptions for all intravenous (IV) medications and infusions to be provided for patients. Assures that the appropriate dose, rate and route of administration have been ordered.
Responds to incoming calls from patients, families, physicians, and referral sources: Provides general information about home infusion services in response to inquiries. Collects sufficient and appropriate referral information to determine a prospective patients service needs.
Supervises technicians to ensure that the final IV product is stable and safe to infuse intravenously.
Dispense IV solutions and ancillary supplies required for the proper infusion of the product.
Ensure that the patient knows the proper storage and use of the drug through instructions from the registered nurse (RN).
Coordinate the transition of the patient from the hospital to the home setting along with the physician, floor nurse, Infusion nurse clinician, and other professionals as needed.
Communicates clearly, sensitively and in a supportive professional manner with patients/family/caregivers, co-workers and the public: Maintains close communications with Hospital Liaisons, hospital based Nurse Clinicians, Hospice Coordinator and other referral sources in relationship to issues around patient referrals, discharge plans, and visit needs.
Provide pertinent drug information to physicians, providers and nursing staff regarding drug therapy.
Required qualifications for this position include:
Bachelor's Degree or PharmD from an accredited school of pharmacy
Current Washington State Registered Pharmacist License
2 years of experience as a Licensed Pharmacist
Preferred qualifications for this position include:
Completion of an ASHP approved residency program
1 year of experience in Long term care, infusion pharmacy, or hospital Pharmacy
Board Certification: Compound Sterile Preparation, Nutrition Support and/or Pharmacotherapy
