Providence Pharmacist - Patient Registration in Tukwila, Washington
Providence is calling a Pharmacist – Infusion Pharmacy (Pharmacy, Day Shift, 1.0 FTE) to Providence Infusion & Pharmacy Services in Tukwila, Washington. This position is responsible for medication distribution and clinical drug services of IV therapies to Home Infusion patients, Specialty Infusions, Infusions at our Ambulatory Infusion Suites (AIS), Hospice Services, and Skilled Nursing Facility patients by providing the best possible drug therapy under the highest professional standards. The pharmacist works closely with nurses, dieticians and pharmacy technicians to ensure that all patients are receiving quality care that is delivered in a safe and efficient manner.
In this position you will:
Interpret orders and/or prescriptions for all intravenous (IV) medications and infusions to be provided for patients. Assures that the appropriate dose, rate and route of administration have been ordered.
Supervises technicians to ensure that the final IV product is stable and safe to infuse intravenously.
Dispense IV solutions and ancillary supplies required for the proper infusion of the product.
Ensure that the patient knows the proper storage and use of the drug through instructions from the registered nurse (RN) or pharmacist.
Coordinate the transition of the patient from the hospital to the home setting along with the physician, floor nurse, Infusion nurse clinician, and other professionals as needed.
Provide counsel to patients, physicians, and nurses as appropriate
Provide pertinent drug information to physicians, providers and nursing staff regarding drug therapy.
Required qualifications for this position include:
Bachelor's Degree or PharmD from an accredited school of pharmacy
Current Washington State Registered Pharmacist License
2 years of experience as a Licensed Pharmacist
Preferred qualifications for this position include:
Completion of an ASHP approved residency program
Home Infusion Experience or Inpatient Pharmacy Experience
Board Certification: Compound Sterile Preparation, Nutrition Support and/or Pharmacotherapy
