The Registered Nurse Case Manager is responsible for providing evaluation and nursing services on an intermittent basis in the patient's place of residence. Functions independently and/or in conjunction with an interdisciplinary team based on the patient's identified needs. Practice is within the scope defined by the Washington State Nurse Practice Act, based on the nursing process model and concept of community health nursing.
In this position you will:
Maintains homecare standards of nursing care and provides care in accordance with policies and procedures. Possesses current clinical skills and demonstrates broad scope of clinical knowledge.
Assesses the patient’s condition, nursing needs, safety and need for additional services on the initial visit. Utilizes that information to initialize a plan of care/treatment with nursing goals and interventions to meet those goals. Assessment will include the physical, psychological, social, and rehabilitative needs of the patient.
Provides assessment and management of the patients care plan in collaboration with interdisciplinary team and provides supervision and ongoing coordination of services and resources as indicated in the plan of care. Recommends and facilitates appropriate medical and/or rehabilitation follow-up once home care services are discontinued.
Participates in agency in-service programs and community continuing education to remain clinically current in nursing practice and homecare.
Works collaboratively with interdisciplinary team to develop, implement, and update patients plan of care/treatment.
Collaborates with physician regarding initial plan of care/treatment and changes that occur in the patients condition and needs.
Instructs and supervises the Home Health Aide based upon a specified plan of care. Supervises HHA POC at a minimum of every 14 days. Updates appropriate members of the interdisciplinary team both verbally and in writing when any status changes occur.
Responds to urgent patient needs by prioritizing and triaging treatments. Communicates schedule and work plan to Supervisor and/or scheduling coordinators daily and other Team members as needed. Provides patient’s information to Clinical Manager/Supervisor and staffing for efficient communication.
Independently manages own caseload, plans and schedules home visits and coordinates with other team members as needed to maximize effectiveness of overall care plan. Organizes weekly schedule to attend required meetings within schedule of patient care needs. Organizes and coordinates transfer of patient’s information for coverage during absences.
Precepts students assigned by the supervisor providing clinical instruction, clinical supervision of care provided and administrative follow-up and coordination. Participates in the orientation of new homecare staff members by orienting them to the unique clinical practice of their discipline in the home setting.
Provides instruction to patients and families in areas identified in the plan of care/treatment in order to promote the optimum independence and health at discharge. Utilizes effective teaching methods and styles as appropriate to audience.
Required qualifications for this position include:
Graduate of an accredited school of nursing.
1 year of full time experience practicing as a Registered Nurse.
Current Washington State Registered Nursing License required.
Current Washington State Driver’s license and have access to daily use of an insured, reliable vehicle.
Current Healthcare Provider CPR certification.
Preferred qualifications for this position include:
1 year of recent (within last three years) clinical experience in a Medicare certified Homecare or Hospice agency/facility doing intermittent visits and case management.
OASIS Certification preferred.
