Social Worker MSW - Hospice Seattle at SCS Hospice

The MSW Social Worker is responsible for providing services to hospice patients, families, and caregivers, addressing a wide range of social work care coordination and psychosocial issues related to end-of-life needs including: adjustment to illness/prognosis, care system issues, community resource issues, insurance/financial issues, legal/ethical issues, and end-of-life planning. This position develops and implements a plan of care in collaboration with the Hospice Interdisciplinary Group (IDG) and is responsible for an assigned caseload shared with an RN Case Manager and a Hospice HA with the goal of promoting patient and family choice, ensuring continuity of care, and enhancing the quality of life of dying patients.

Required qualifications for this position include:

  • Masters Degree from an accredited School of Social Work

  • Current Social Work Certification or License through Washington State Department of Health

  • Current Washington State Driver's license and automobile insurance

  • Current Healthcare Provider CPR certification

Preferred qualifications:

  • 1 year prior internship experience in a Providence Health System setting

  • 1 year prior experience in a healthcare setting

Providence Hospice of Seattle is a not-for-profit, community-based hospice ministry serving patients in their places of residence throughout King, and Pierce Counties in Washington state. Our skilled and compassionate team includes nurses, medical social workers, home health aides, chaplains, occupational/physical therapists, medical directors, grief support counselors and volunteers. We provide quality, timely, and comprehensive physical, emotional and spiritual care to patients at the end of life and their families.
