Providence is calling a Social Worker (MSW) (Hospice, Day Shift, 1.0 FTE) to Providence Hospice of Seattle in Tukwila, Washington.

This is a full-time position which will include benefits.
In this position you will:
Assess initially and on ongoing basis the needs of terminally ill hospice patients and families, addressing psychosocial issues.
Develop and implement a plan of care in collaboration with the Hospice Interdisciplinary Group (IDG).
Appropriately collaborate with Hospice Interdisciplinary Group (IDG), including attending physician, to create and update the plan of care and to optimize the care provided to patient and family.
Assess, on an ongoing basis, appropriateness of Hospice services for each patient/eligibility for continuing Hospice services. Coordinates with physician, Hospice Interdisciplinary Group (IDG), patient, family, and others when a patient’s discharge from hospice is indicated.
Plan Hospice Social Worker interventions as part of Hospice Interdisciplinary Group (IDG) care plan, anticipate expected changes and maximize quality of life. Collaborate with Hospice Interdisciplinary Group (IDG) members, including bereavement, and coordinates appropriately with community resources.
Evaluate and document patient/family response to interventions, revising care plan appropriately
Required qualifications for this position include:
Master’s degree in Social Work (MSW) from an accredited School of Social Work
Current Social Work Certification or License through Washington State Department of Health
Current Healthcare Provider Cardiopulmonary Resuscitation (CPR) certification
Current Washington State Driver's license and automobile insurance
1 year of experience in a health care setting
2 years post master's work experience; prior internship at PHOS or other relevant social services-related role counts towards the 3 years of post master’s work
Preferred qualifications for this position include:
Prior experience in hospice care setting and/or case management
2 years of experience in a health care setting
