Providence is calling a Social Worker (MSW) (Hospice, Day Shift, 1.0 FTE) to Providence Hospice of Seattle in Tukwila, Washington.

The territory for this position will include North Seattle, Shoreline, and Lake Forest Park (zip codes of 98155, 98133, and 98177).
In this position you will:
Assess initially and on ongoing basis the needs of terminally ill hospice patients and families, addressing psychosocial issues.
Develop and implement a plan of care in collaboration with the Hospice Interdisciplinary Group (IDG).
Appropriately collaborate with Hospice Interdisciplinary Group (IDG), including attending physician, to create and update the plan of care and to optimize the care provided to patient and family.
Assess, on an ongoing basis, appropriateness of Hospice services for each patient/eligibility for continuing Hospice services. Coordinate with physician, Hospice Interdisciplinary Group (IDG), patient, family, and others when a patient’s discharge from hospice is indicated.
Plan Hospice Social Worker interventions as part of Hospice Interdisciplinary Group (IDG) care plan, anticipate expected changes and maximize quality of life. Collaborate with Hospice Interdisciplinary Group (IDG) members, including bereavement, and coordinate appropriately with community resources.
Evaluate and document patient/family response to interventions, revising care plan appropriately.
Required qualifications for this position include:
Master’s degree in Social Work (MSW) from an accredited School of Social Work
Current Social Work Certification or License through Washington State Department of Health
Current Healthcare Provider Cardiopulmonary Resuscitation (CPR) certification
Current Washington State Driver's license and automobile insurance
1 year of Post-Masters work experience in a health care setting
1 year prior internship experience in a Providence Health System hospice or palliative care setting may be substituted for the required one year post-Masters work in a health care setting
Preferred qualifications for this position include:
